According to the 1864 West Virginia Adjutant General's Report, John Fordyce, 22, mustered into service 12 Sep 1861, Company "F;" he served as a sergeant (4th). He was commissioned captain on 8 Jan 1863 and transferred to Company "A". Mustered out expiration term of service, 26 Nov 1864.

On the website, The Civil War Medals of West Virginia...Where Are They, Captain Fordyce's medal is listed; the owner is Dr. Al Harley.

Captain John Fordyce received a government pension, certificate #330129. His widow, Anna, applied March 1917(?) and her certificate was #657613.
