Declaration for ORIGINAL INVALID PENSION

State of OHIO
County of WASHINGTON

On this 29 day of JANUARY, AD one thousand eight hundred and SEVENTY-
NINE personally appeared before me, Judge of the Probate Court, a 
court of record within and for the County and State aforesaid GEORGE 
ATKINSON aged 36 years, a resident of the vicinity of FIFTEEN, county 
of WASHINGTON, State of OHIO, who, being duly sworn according to law, 
declares that he is the identical GEORGE ATKINSON who was enrolled on 
the 27th of FEB, 1864, in Company "F" of the 36th Regiment of the 
OHIO VOLS commanded by Captain Joseph Kelley and was honorably 
discharged at Columbus, O on the 27th day of JULY, 1865;  that his 
personal description is as follows;  Age, 36 YEARS;  height 5 FEET 8 
INCHES; complexion, FAIR;  hair DARK; eyes, HAZEL.  That while a 
member of the organization aforesaid, in the service and in the line 
of his duty at NEAR CEDAR CREEK AND NOT FAR FROM STRASBURG, in the 
state of VA on or about the 12 day of AUGUST, 1864, he was WOUNDED IN 
ACTION BY A GUN SHOT IN THE RIGHT ANKLE- TOTALLY DISABLING HIM AT THE 
TIME.

That he was treated in hospitals as follows:  In G.H. called --- at 
BALTIMORE, MD from about AUGUST 16, 1864 to about SEPTEMBER 9, 1864.  
In G. Hosp. at PHILADELPHIA, PA, called CHESTNUT HILL, from SEPTEMBER 
10- 64 to about SEPTEMBER 28- 1864.

That he has NOT  been employed in the military or naval service 
otherwise than as stated above, either before FEB 27 - 1864 nor since 
JULY 27, 1865.

That since leaving the service this applicant has resided in the 
COUNTY of WASHINGTON in the state of OHIO,and his occupation has been 
that of a FARMER.  That prior to his entry into the service above 
named he was a man of good, sound, physical health, being when 
enrolled a FARMER.  That he is now ONE HALF disabled from obtaining 
his subsistence by manual labor by reason of his injuries, above 
described, received in the service of the United States;  and he 
therefore makes this declaration for the purpose of being placed on 
the invalid pension roll of the United States.  

He hereby appoints, with full power of substitution and revocation, A. 
W. McCormick, of Cincinnati, O., his true and lawful attorney to 
prosecute his claim.  That he has NOT received NOR applied for a 
Pension.  That his Post Office Address is FIFTEEN county of 
WASHINGTON, State of OHIO.

ATKINSON, the claimant, sign his name  (or make his mark) to the 
foregoing declaration;  that they have every reason to believe, from 
the appearance of said claimant and their acquaintance with him, that 
he is the identical person he represents himself to be;  and that 
they have no interest in the prosecution of this claim.

SUPPORTING AFFIDAVIT:

On the 4th day of MARCH 1879, before the undersigned authority within 
and for said County, and State, personally appeared JOSEPH SMITH, who 
being duly sworn, states on oath that GEORGE ATKINSON OF CO F 36TH 
OHIO INF VOLS AT NEAR CEDAR CREEK AND NOT FAR FROM STRASBURG VA ABOUT 
AUG 12-1864 WAS WOUNDED IN ACTION BY A GUNSHOT IN THE LEFT LEG BELOW 
THE KNEE AND ALSO BY A GUNSHOT IN THE RIGHT ANKLE - TOTALLY DISABLING 
HIM AT THAT TIME.

THAT AFFIANT WAS A MEMBER OF SAID COMPANY AND SERVING WITH SAID 
ATKINSON AT THE TIME.

I have the honor to acknowledge the receipt from your office an 
application for Pension No. 265896, and to return it herewith, with 
such information as is furnished by the files of this Office.

It appears from the ENLISTMENT papers on file in this Office that 
GEORGE T. ATKINSON was enrolled and mustered into service as a 
PRIVATE on the 27th day of FEB, 1864, at MARIETTA, OHIO, in Company 
"F", 36th Regiment of OHIO Volunteers, to serve 3 years, or during 
the war. On the Muster Roll of Co. "F", of that Regiment, for the 
months of MAY & JUNE, 1864, he is reported PRESENT.  JULY & 
AUG 1864 REPORTED GEORGE ATKINSON WOUNDED AUGUST 15, 1864.  SENT TO 
HOSPITAL.  REGT WAS IN ACTION AT CEDAR CREEK, VA, THAT DATE.  
SEPTEMBER & OCTOBER 1864 PRESENT AND SO BORNE TO JUNE 30, 1865.  
MUSTERED OUT WITH COMPANY AT WHEELING, WV JULY 27, 1865.  CAS LIST OF 
2nd DIV A of WV REPORTS HIM WOUNDED IN ACTION AUG 15, 1864.  NATURE 
AND LOCATION OF WOUNDS NOT GIVEN.

Declaration for INCREASE of an Invalid Pension

State of WEST VIRGINIA
County of RITCHIE

On this 3rd day of NOVEMBER, A.D. one thousand eight hundred and 
EIGHTY-SIX personally appeared before me G. W. Amos Clerk of the 
County Court the same being within and for the County and State 
aforesaid, GEORGE ATKINSON aged 40 years, a resident of Goose Creek, 
County of RITCHIE State of W.Va. who being duly sworn according to 
law, declares that he is a pensioner of the United States, duly 
enrolled at the KNOXVILLE Pension Agency at the rate of $2 per month 
by reason of disability incurred in the military service of the 
United States while (1) in Co. F 36 Reg. OHIO Vol's that his present 
physical condition is such that he believes himself entitled to 
receive an increased pension;  and that his present Pension 
Certificate is No. 175.572....

He further declares that he is disabled in the following manner, to 
wit:  HIS GUNSHOT WOUND OF LEFT LEG & RIGHT ANKLE SO DISABLE HIM 
THAT HE BELIEVES HE SHOULD HAVE A HIGHER RATE.

He claims pension for all effects of the above, and believes he 
should have $8.00 a month pension for the future;   that he appoints 
A.W. McCormick & Son of Cincinnati, Ohio, his true and lawful 
attorneys to prosecute his claim;  that his residence is as above, 
and his post office is GOOSE CREEK, RITCHIE COUNTY, WV.        

Witnesses who can write:  L.A. Bennington & W. G. Cunningham

Also personally appeared W. G. CUNNINGHAM residing at GOOSE CREEK 
RITCHIE COUNTY, WV and THOS E. DAVIS residing at RITCHIE COUNTY, WV, 
persons whom I certify to be respectable and entitled to credit, and 
who, being duly sworn, say they were present and saw GEORGE ATKINSON, 
the claimant, sign his name (or make his mark) to the foregoing 
declaration; that they have every reason to believe, from the

Affidavit of Neighbors, Employers or those who worked with Soldier

State of West Virginia, RITCHIE County, ss.

In the matter of the Pension Claim No. 175572 of GEORGE ATKINSON, Co 
"F Regt 36 OHIO Vols:

On the 20 day of FEBRUARY 1890, appeared before the undersigned 
authority within and for said County and State personally appeared 
HARVEY BOWIE and ELI C. BOWIE, who, being duly sworn, state on oath 
that they first became acquainted with the physical condition of 
GEORGE ATKINSON after his return from the army, about AUGUST 1865, 
and found him then suffering from disability or disease called 
CHRONIC DIARRHEA from which they knew him to continue to suffer each 
year up to about FEBRUARY 1890.

The progress and symptoms of this disease have been as follows:
THAT THEY LIVED NEAR HIM, EXCEPTING ABOUT TWO YEARS,UP TO THE PRESENT 
TIME.  AT TIMES THE DISEASE HAS DISABLED THE SOLDIER SO AS TO UNFIT 
HIM VERY GREATLY FOR MANUAL LABOR- AMOUNTING TO TOTAL DISABILITY- 
THEN AT OTHER TIMES HE WOULD APPEAR TO BE BETTER.  AT NO TIME HAS HE 
BEEN ABLE TO DO ANYTHING LIKE FULL MANUAL LABOR.

That their means of knowing his condition are as follows:  LIVING 
NEAR HIM, OBSERVING HIS DAILY LIFE, AND HAVING THE BEST OPPORTUNITIES 
FOR KNOWING HIS PHYSICAL CONDITION.  AFFIANTS SERVED IN SAME CO AND 
REGT WITH THE SOLDIER.

That claimant's occupation was that of a FARMER.....  That in the 
affiant's opinion he was disabled for hard work to a 3/4 degree on an 
average during the above period, and to a *3/4 degree at present, by 
the above, as near as they can judge.

That they testify from personal knowledge, and have no interest in 
Pension Claim.

BOWIE, GOOSE CREEK, WV.

Declaration for INVALID PENSION, Act of June 27, 1890.

State of West Virginia, County of Ritchie ss.

 On this 10 day of AUGUST A.D., one thousand eight hundred and ninety 
TWO personally appeared before me, a Notary Public within and for the 
County and State aforesaid, GEORGE ATKINSON aged 49 YEARS, a resident 
of GOOSE CREEK County of RITCHIE State of WV, who being duly sworn 
according to law, declares that he is the identical  GEORGE ATKINSON 
who was enrolled on the 27th day of FEBRUARY, 1864 in CO F. 36th OHIO 
VOL. in the War of the Rebellion and served at least 90 days, and was 
honorably discharged at WHEELING, WV on 27 day of JULY 1865.

That he is totally unable to  earn a support by reason of GUN SHOT WOUND 
OF LEFT LEG AND RIGHT ANKLE, CHRONIC DIARRHEA, RHEUMATISM, DISEASE OF 
HEART, EFFECTS OF CAMP FEVER AND GENERAL DEBILITY.

That said debilities are not due to his vicious habits, and are to 
the best of his knowledge and belief permanent....  That he is a 
pensioner under Certificate No. 175,572 at $4 per month for G.S.W. OF 
LEFT LEG AND RIGHT ANKLE.

That he has not been in the military or naval service of the United 
States since his discharge from the above named service JULY 27 1865.

That he makes this declaration for the purpose of being placed on the 
pension roll of the United States under the provisions of the act of 
JUNE 27, 1890.  He hereby appoints A.W. McCormick & Sons of 
Cincinnati, O., his true and lawful attorneys to prosecute his 
claims, the fee to be Ten dollars, as prescribed by law, in cases 
under this Act.

That his address is GOOSE CREEK, County of RITCHIE, State of WV.

Affidavit for General Purposes

State of WEST VA RITCHIE COUNTY, ss:

In the matter of the Pension Cef No 175572 of GEORGE ATKINSON Co F 36 
Regt. Ohio Inf Vols.:

On the 4 day of March 1899,before the undersgned authority within and 
for said County and State, personally appeared GEORGE ATKINSON THE 
CLAIMANT aged 53 years who, being duly sworn, states on oath that HE 
RECEIVED GUNSHOT WOUND OF RIGHT ANKLE AT NEAR STRASBURG ABOUT AUGT 12 
1864 UNDER FOLLOWING CIRCUMSTANCES.  WHILE IN LINE WASE DETAILED TOO 
CARRY WATTER.  WHEN RETURNED WE WAS ATTACKED AND I WAS WONEDED IN 
LEFT LEGE RIGHT ANKLE.  THAT THE SAME WAS NOT CAUSED BY VICIOUS 
HABITS.

one thousand nine hundred and eight personly appeared before me and 
Within and for the said County and State aforesaid.  George Atkinson 
who being duly sworn according to law declares that he is 62 years of 
age and a Resident Combs, County of Ritchie State of West Va and that 
he is the identical person who was enrolled at Marietta, Ohio 
February 27/1864 as a Privat in Co F- 36 regt Ohio Vol Infantry in 
the Service of the United States in the Civil War and was Honorably 
discharged at Wheeling, WV on the 27th day of July 1865.  That he was 
employed in the military or naval Service of the United States 
Otherwise as stated above, that his personal description at 
enlistment was as follows.  Hight 5 ft inches, Complection fair 
Collar of eyes hazel dark hair tht his occupation was farmer that he 
was born October 20/1846 at Washington County Ohio that his 
sevrlplaces of residence since leaving the service have been as 
follows  Washington County from 1865-1880 and from 1880 to 1908 in 
the state of West Va in Ritchie County.  That he is a pensioner under 
ctf # 175572 at $6.00 pr month a date of Certifict July 26, 1887 that 
he makes this declaration for the purpose of being placed on the 
pension roll of the United States under the Provision of the act of 
February 6/1997.

On this 13th day of Jan A.D. one thousand nine hundred and 13, 
personally appeared before me, a GEORGE ATKINSON within and for the 
county and State aforesaid, who, being duly sworn according to law, 
declares that he is --- years of age and a resident of the county of 
RITCHIE, State of West Virginia;  and that he is the identical person 
who was enrolled at MARIETTA, OHIO, under the name of George 
Atkinson, on th 27 day of FEBRUARY, 1864 as a PRIVATE, in CO F 36 
OHIO INFANTRY PRIVATE in the Service of the United States, in the 
CIVIL war, and was honorably discharged at WHEELING WV on the 28th 
day of JULY, 1865.
 
That he was not employed in the military or naval service of the 
United States other than as stated above.  That his personal 
description at enlistment was as follows:  Height, 5 FEET 8 INCHES;  
complexion, FAIR;  color of eyes, HAZEL;  color of hair, DARK;  that 
his occupation was FARMER;  that he was born OCTOBER 20, 1846, at 
WASHINGTON COUNTY, OHIO.

That his several places of residence since leaving the service have 
been as follows:  WASHINGTON COUNTY OHIO AND RITCHIE COUNTY WVa MOVED 
FROM OHIO TO WEST VA

That he is pensioner under certificate No. 175572.  That he has 
(erased) applied for pension under original No 175572 when 62 years 
of age.

That he makes this declaration for the purpose of being placed on the 
pension roll of the United States wunder the provisions of the act of 
May 11, 1912.

That his post office address is COMBS, county of RITCHIE State of 
WVa.
