HERMINA LILLIAN HORAK

Hermina Lillian Horak was born on September 28, 1911 in Bentleyville, Washington County, Pennsylvania. Her parents were Joseph T. Horak and Methodia Lisak, both born in Moravia.

At the time of the 1920 Federal Census, the family resided on Main Street, Bentleyville. The children in the home were Mary, 11, Helen, 10, Hermina, 8, Joe, 7, Frank, 5, Annie, 2, and Cecelia, 2 months.

On December 2, 1935, in Brooke County, West Virginia, Hermina Horak, 24, born in Bentleyville, married Nelson Dwight Willard, 38, born in Vermont. Both were residing in Pittsburgh.

The fate of the husband and marriage are unknown, but Hermina Lillian Horak was engaged in Postgraduate study (Supervision) at Columbia Hospital, Wilkinsburg, Pennsylvania, when she participated in the U. S. World War II Nurse Cadet Corps in 1944.

Hermina died on March 15, 1989. Last residence, Bentleyville.
