     Washington, August 10, 1882
     Respectfully returned to the Commissioner of Pensions. Daniel S. Haymond, a private of Company ”H” 12 Regiment Virginia Volunteers, was enrolled on the 16 day of August, 1862, at Grafton Va for 3 yrs, and is reported: on roll from enlistment to Oct 31, /62 present Nov and Dec /62 to Feb 28/63 present. Muster-out roll of Co. dated June 16 /65 reports him discharged for disability at Winchester, Va (no date given). Regimental return November /62 reports him left in Grafton Hospital November 19. Certificate of Disability shows him discharged at Winchester Va. Feby, 26th 1863 The records of this office do not show disabilities alleged. Cause for which left in hospital not stated, Regtt. Hospl records not on file.

CERTIFICATE OF DISABILITY FOR DISCHARGE

Daniel S. Haymond, of Captain J.H. Bristow’s company, H of the twelfth Va Regiment of the United States Infty, was enlisted by J.H. Bristow of the twelfth Regiment of Va Vol. Infty, at Grafton, Va on the sixteenth day of August, 1862 to serve three years; he was born in Taylor County in the State of Virginia, is twenty four years of age, five feet eight inches high, dark complexion, gray eyes, dark hair, and by occupation when enlisted a farmer. During the last two months said soldier has been unfit for duty 60 days. The said Haymond, has been afflicted with something like the “Chronic Asthma” ever since the 25th August 1862.

     I certify that I have carefully examined the said Daniel S. Haymond, of Captain J.H. Bristow’s company, and fine him incapable of performing the duties of a soldier because of Phthisic Pulminatis existing from 1st Sept. 1862 of continuing and getting worse ever since has had pulmonary symptoms before he joined the army -- Degree of disability______

DECLARATION FOR AN ORIGINAL INVALID PENSION
STATE OF West Virginia
COUNTY OF Taylor
On this 2nd day of October, A.D. one thousand eight hundred and seventy nine personally appeared before me Clerk of the County court a Court of Record within and for the county and State aforesaid, Daniel S. Haymond, aged 40 years, who, being duly sworn according to law, declares that he is the identical Daniel S. Haymond, who was ENROLLED on the 16TH DAY OF August, 1862, in company, H of the twelfth Regiment of the Va Vol. Infty, commanded by Captain J.H. Bristow and was honorably DISCHARGED at Winchester Va.
On the 26th day of February , 1863, that the personal description is as follows: age 24 years of age, five feet eight inches, complexion, dark; hair, dark; eyes, gray. That while a member of the organization aforesaid, in the service and in the line of his duty at Beverly, in the state of West Virginia on or about the 1st day of November 1862 he contracted the measles which settled on his lungs. That he was treated in hospitals as follows: at Grafton, West Virginia during the month of November 1862. That he has never been employed in the military or naval service otherwise that as stated above. That since leaving the service this applicant has resided in the counties of Taylor and Randolph in the State of West Virginia and his occupation has been that of a Physician. Prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a Farmer. That he is now almost entirely disabled from obtaining his subsistence by manual labor, by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints with full power of substitution and revocation his true and lawful attorney to prosecute his claim. That he has never received nor applied for a pension; That his residence is Leedsville, Randolph Co. W Va
