Declaration For Original Invalid Pension

State of SOUTH DAKOTA
County of HAND, ss:
     On this 20 day of January, A.D. one thousand eight hundred and EIGHTY SEVEN(?), personally appeared before me a Probate Judge, of the Probate Court, a court of record within and for the county and territory aforesaid, ISAAC N. FISH aged 37 years, a resident of CEDAR P.O., County of HAND, and State of DAKOTA TY, who, being duly sworn according to law, declares that he is the identical ISAAC N. FISH, who was enrolled on the 16th day of AUGUST, 1862, in Company C of the 12" regiment of WEST VA INF VOL commanded by ERASTUS G. BARTLETT, and was honorably discharged at RICHMOND, VA on 16" JUNE, 1865; that his personal description is as follows: Age, 37 years; height, 6 feet; complexion, FAIR; hair, LIGHT; eyes, BLUE. That while a member of the organization aforesaid, in the service, and in the line of his duty at NEAR CROSS KEYS, in the State of VIRGINIA on or about the 12 day of JUNE, 1864, he WAS WOUNDED BY A GUNSHOT THROUGH THE MIDDLE KNUCKLE OF THE SECOND FINGER OF THE RIGHT HAND.
That he has not been employed in the military or naval service otherwise than as stated above.
That since leaving the service this applicant has resided near the CITY of DECATOR in the State of ILLINOIS AND NEAR CEDAR P.O. HAND COUNTY DAKOTA, and his occupation has been that of a FARMER.
That prior to his entry into the service above named, he was a man of good, sound, physical health, being when enrolled a FARMER. That he is now PARTIALLY disabled from obtaining his subsistence by manuel labor by reason of his injuries above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension-roll of the United States.

He hereby appoints with full power of substitution and revocation, S. J. CHRIST, of RU(?) HEIGHTS, DAK TY, his true and lawful attorney, to prosecute his claim. That he has not received or applied for a pension. That his Post Office Address is CEDAR, county of HAND DAKOTA TY.

     On this 7th day of April, A.D. one thousand eight hundred and NINETY ONE, personally appeared before me, RICHARD NEEDHAM, a Justice of the Peace in and for the county and State aforesaid, I. N. FISH aged 42 years, a resident of CEDAR, County of HAND, and State of SOUTH DAKOTA, who, being duly sworn according to law, declares that he is the identical person who was entered service during the War of the Rebellion under the name of I. N. FISH on or about the 1st day of AUG 1862, as a PRIVATE, in Company C of the 12th regiment of WEST VIRGINIA commanded by E. G. BARTLETT, and was honorably discharged at WHEELING, WEST VIRGINIA on or about the FIFTH day of JUNE, 1865, by reason of TERMINATION OF THE WAR; that his personal description is as follows: Age, 42 years; height, 5 feet 10 inches; complexion, FAIR; hair, LIGHT; eyes, BLUE. That he is now suffering from BRONCHITIS RHEUMATISM, PILES, A DEFORMITY OF SECOND FINGER OF RIGHT HAND FROM GUN SHOT WOUND WHICH SERIOUSLY INTERFERED WITH THE USE OF RIGHT HAND ALSO CATARRAH OF THE HEAD AND GENERAL BREAKING DOWN OF VITAL ENERGIES.
That since the fifth day of JUNE, A.D. 1865, he has not been employed in the military or naval service of the United States.

He hereby appoints with full power of substitution and revocation, GEORGE E. LEMON, of Washington, D.C., his true and lawful Attorney, to prosecute his claim. That he has received ??? applied for a pension. 544279.
That his Postoffice address is CEDAR, County of HAND, State of SOUTH DAKOTA

Declaration For Pension

State of ILLINOIS, County of JASPER, ss:
     On this 5th day of December, A.D. one thousand nine hundred and EIGHT, personally appeared before me, --- within and for the county and State aforesaid, ISAAC N. FISH who, being duly sworn according to law, declares that he is 62 years of age, and a resident of County of JASPER, State of ILLINOIS; and that he is the identical person who was enrolled at WEST VIRGINIA under the name of ISAAC N. FISH, on the 16 day of AUG 1862, as a PRIVATE, in Company C 12th WEST VIRGINIA VOL INFT in the service of the United States, in the Civil war, and was honorably discharged at RICHMOND, V on the 16th day of JUNE 1865.
That he served IN NO OTHER SERVICE.
That he was not employed in the military or naval service of the United States otherwise than as stated above.
That his personal description at enlistment was as follows: Height:, 5 feet 6 inches; complexion, FAIR; color of eyes, BLUE; color of hair, FAIR; that his occupation was FARMER; that he was born NOVEMBER 27 1846, at ROM's(?) RIDGE, WEST VIRGINIA.
     That his several places of residence since leaving the service have been as follows: ABOUT NINE YEARS IN SOUTH DAKOTA. REMAINDER IN LOGAN COUNTY ILLINOIS
     That he is NOW a pensioner. That he has heretofore applied for a pension AND IS DRAWING $8.00 PER MONTH UNDER CERTIFICATE #544279.
     That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of Feb 6, 1907.
     That his post-office address is WEST LIBERTY, county of JASPER State of ILLINOIS.
