Application of Discharged Soldiers for Additional Bounty

State of West Virginia County of Ohio ss.
On this 30th day of November 1866 personally appeared before me a clerk of the circuit court in and for the County and State aforesaid John W. Harbinson, of the county of Marshall and the State of West Virginia personally known to me who being duly sworn according to law, declares that his age is twenty three years that is a resident of ___County of Marshall State of West Virginia, and that he is the identical John W. Harbinson who was enlisted as a corporal in Company B of the Twelefth Regiment of West Va. Infantry to serve for the period of three years and was discharged from the service of the United States as a Corporal at Richmond, Virginia, on the 16th day of June 1865 by reason of General Order from War Department dated May 27, 1865 and that he was again enlisted as a ___in Company___, of the ___ Regiment of ____to serve for a period of____ and that he was discharged from the service of the United States as a _____ ___on the ___day of ___186_ by reason of _____ and that there is additional Bounty of $One Hundred due him under the act of Congress approved July 28, 1866. And he does further declare that he has not bartered, sold, assigned, transferred, loaned, exchanged, or given away his final discharge papers, or any interestin the bounty provided by this or any other Act of Congress that he has not already received or is entitled to received any other or greater bounty than $100, and that the statement of service above given is a correct and true statement of any and all service rendered by him during the rebellion, and that he has never served otherwise than as stated. He hereby appoints, acknowledges, and authorizes GEO. B. CALDWELL of Wheeling, Ohio County, West Virginia, as his true and lawful Attorney, for him and his name and stead, to present and procure payment of the foregoing claim for Bounty, and to receive and receipt for whatever may be issued in satisfaction of said claim, and to da any any all lawful acts necessary to effect the object of said appointment. And he desires any communications concerning this Claim to be sent to his aforesaid Attorney, at Wheeling Ohio County, West Virginia That his own Post Office address is Woodland County of Marshall State of West Virginia.

left inguinal hernia nasal pharyngeal catarrh, dis. of stomach, bowels, rectum, rheumatism, ? dis. of heart, gen'l debility, deafness, dis. of breast lungs. He receives a pension of 10.00 dollars per month. He makes the following statement in regard to the original of his disabilities and date when first discovered by him: same as above

Birthplace, Marshall Co. W. Va.; age 62 years; height, 5-10 weight, 155 pounds; complexion, dark; color of eyes, brown; color of hair, gray & brown; occupation, Carpenter; permenant marks and scars other than those described below, none We hereby certify that upon examination we find the following objective conditions: Pulse rate, 100, 132, 148; respiration, 20, 22, 28; temperature, 98.6

There is a left complete direct inguinal hernia. Tumor passes out of external ring and descends into scrotum, measuring 6 by 4 inches. Tips of two fingers readily enter the ring. It is reducible and well retained by truss. There is some post nasal catarrh; turbinals are enlarged, and there is a marked degree of deafness. Claimant cannot hear ordinary conversation at 10 ft., but can at 6 ft. There is a constant roaring noise in the head, and drum heads are retracted. Eustachin tubes are pervious.

Heart-Sounds are diminished, and at apex there is a soft systolic blowing sound which is not transmitted to any great extent. The first sound over the base is diminished, second, slightly accentuated. Rhythm is bad; action is extremely rapid and dulness extends to nipple line. There is no cyanosis or eodema, but great dyspnoea, more especially on active exercise. This man is suffering from myocardial degeneration. Arteries are slightly sclerosed

There is some gastroentosis and some tenderness of epigastrium. Tongue is broad, red and fissured. Some tympanites of colon. Rectum is slightly injected. No piles, fissures, fistula or prolapse.

The shoulders are stiffened and motion only moderately impaired. Back is somewhat stiffened; motion impaired about one-forth. All other joints are in a healthy condition. Claimant suffers some from muscular rheumatism. At present there is no objective evidence of same.

Over the lower portion of the left lung, at base, there are a few soft, moist rales. The chest expansion is 32 1/2" to 35 1/2". No dullness on percussion. No objective evidence of disease of breast.

Urine - highly colored, specific gravity, 1026, no albumen or sugar.

No evidence of vicious habits; no other disability found, except those arising from advancing years.

We find that the aggregate permenant disability for earning a support by manual labor to be due to left inguinal hernia, disease of heart and arteries, and disturbances of distrubance organs, not due to vicious habits, and warrants a rating of $12.00

On this 27 day of May A.D. one thousand nine hundred and twelve, personaly appeared before me a Notary Public within and for the county and State aforesaid; John W. Harbinson who, being duly sworn according to law, declares that he is 68 years of age, and a resident of Cleveland, county of Cayahoga State of Ohio; and that he is the identical person who was ENROLLED at Wheeling W.Va., under the name of John W. Harbinson on the 11 day of August, 1862 as a private, in CO B. 12th West Va. volunter Infantry in the service of the United States, in the Civil War, and was HONARABLY DISCHARGED

That he also served ________ That he was not employed in the military or Naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height 5 feet 10 inches; complexion, fair; color of eyes, gray; color of hair, light; that his occupation was a farmer I was born December 23, 1843 at Marshall County West. Va. That his several places of residence since leaving the service have been as follows: Wheeling West Va. in 1869. Came to Cleveland in Spring of 1901.

That he is pensioner under certificate No. 595.818. That he has ____ applied for pension under original No.____

That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.

In the matter of the claim for increase pension of John W. Harmison, who served as John W. Harbinson, in Co. "B", 12th, regt. West Virginia Inf. Vols. Personally came before me, a Notary Public in and for the County and State aforesaid John W. Harmison, the claimant, whose post-office address is: #10406 Euclid Ave., Cleveland, Ohio. a person of lawful age, who being sworn, declares in relation to the aforesaid claim, as follows

I am the identical person who served as corporal in Co."B" 12th. Regt. West Virginia Infantry under the name of John W. Harbinson. As to heretofore stated by me my correct name is John W. Harbinson, and that is the name I enlisted under, but through error on part of the enlisting officermy name was entered on the records as John W. Harbinson and my name was carried through the records as Harbinson, and my original army Discharge which is in my possession was made out in the name of Harbinson. I never paid much attention to the change in my name until recently. When I made my application for pension I made same under the name that is on my Discharge paper. I am told now that I should have called attention to the mistake than, but I did not attach any importance to the error.

Whether I shall at this late day be able to furnish any comrades testimony to my identity is hard to say, its a great many years since I have met any of my comrades. I expect to visit the neighborhood in which I enlisted and if there are any of my old army comrades living I shall have no difficulty in getting their testimony.

On this 10th day of February, A.D. 1925, personally appeared before me a Notary Public, within and for the county and State aforesaid, John W. Harmison alias Harbinson who, being duly sworn according to law, declares that he is 78 years of age, and a resident of Bellaire county of Belmont, State of Ohio; and that he is the identical person who was ENLISTED at Whelling, West Va., under the name of John W. Harbinson on the 11th day of August, 1862 as a corporal in Co. "B" 12th West Va. Infantry Vols has no house, lives with daughter. wife dead in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Richmond, Va., on the 16th of June, 1865 That he served no other Service Had no children in World War, not recieveing any compensation, for any one who was. Only his own pension $50 a month

That his personal description at enlistment was as follows: Height 5 feet 10 inches; complexion, fair color of eyes, gray; color of hair, light; that his occupation was farmer; that he was born December 23rd, 1843, at Marshall County West Va.

That he requires the regular personal aid and attendance of another person on account of the following disabilities:
Bad Rupture on left side, the right also affected Rheumatism, ?? piles, Bad eye sight, and hearing, few old teeth, kidney, Bladder and stomach trouble Indigestion and dizzyness. Has not worked for three years Since January 7th, 1919. I have had to have the regular aid and personal attention of another person especially in dressing on account of age and above disabilities and also a very bad injury to his back.

That since leaving the service he has resided at Woodland, Wheeling, West Va. and Cleveland, Ohio and Bellaire, Ohio. and his occupation has been carpenter That he has ____ applied for pension under Original No. __does not know__ That he is____ a pensioner under Certificate No 595.818. asks to be examined bt Wheeling West Va. medicaL board, it's most concerned

He makes the following statement in regard to the origin of his disabilities and date when first discovered by him: In the summer of 1864 developed hernia on left side. Also had measles during active service confined to Hospital. Since discharge has developed impairment of hearing and vision; now complains of stiffness in back and back of neck; shortness of breath, gas formation in stomach, swelling and stiffness of lower extremities.

Birthplace, Marshall Co., West Va.; age, 82 years; height,70"; weight, 165 pounds; complextion, Dark; color of eyes, Grey; color of hair, Grey; occupation, none unable; permanent marks and scars other than those described below,__________________ It is hereby certified that upon examination the following objective conditions are found: Pulse rate, 78 unable to procure; respiration, 20; temperature, N;

HEAD: Negative throughout
EARS: slight deafness bilateral; no mastoid tenderness, no discharges.
EYES: Increased lacromation. Chronic conjunctivitis; Arcus senilis very marked, vision 20-60 left 20-50 rt.
MOUTH: Few remaining old necrotic dentures; tongue coated, naso pharaynx negative.
NECK: Icreased pulsations of Great blood vessels, no Thyroid pathology, no palpable glands.
CHEST: Flat and Rounded
LUNGS: Respiratory excursion free and equal on both sides, no pathological areas of dulness, no rales.
HEART: PMI in normal position, no definite enlargement or dialation; no murmurs; rate is slightly increased compensation in tact.
ABDOMAN: is placid, showing dialated colon prolapsed. In the left inguinal region there is a hugh mass protruding from external ring, size of a cocanut, unreduceable, very tender and irritated. Right inguinal region negative. There are moderate sized external hemorriods, giving some discomfort; no evidence of bleeding at present.
Extremities: There is moderate stiffness of all joints of lower extremities with varicosities bilateral size of lead pencil.

There is marked general weakness, with loss of flesh. Claimant has difficulty in locomotion. General debility is very marked.

It is the opinion of the board that above disabilities are not the result of vicious habits.

After due consideration and reviewing of the clinical findings the Board does recommend a rating of $72.00 due to defective vision, and hearing; large inguinal hernia, very marked general weakness and the fact that he has to have help in dressing and undressing he being unable to accomplish same himself altho he is able to answer the calls of nature.
