Declaration For Pension

State of WEST VIRGINIA, County of MARSHALL, ss:
     On this 5th day of December, A.D. one thousand nine hundred and TWELVE, personally appeared before me, a Notary Public within and for the county and State aforesaid, JESSE BANE who, being duly sworn according to law, declares that he is 70 years of age, and a resident of CAMERON DISTRICT, county of MARSHALL, State of WEST VIRGINIA; and that he is the identical person who was enrolled at WHEELING, under the name of JESSE BANE, on the 14th day of AUGUST 1862, as a PRIVATE, in Company C 12th WEST VIRGINIA VOL INFT in the service of the United States, in the Civil war, and was honorably discharged at FORT MONROE, on the 17th day of JUNE 1865.
That he served IN NO OTHER SERVICE.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height:, 5 feet 10 inches; complexion, DARK; color of eyes, GREY; color of hair, DARK; that his occupation was FARMER; that he was born DECEMBER 5th 1842, at MARSHALL COUNTY WEST VIRGINIA.
     That his several places of residence since leaving the service have been as follows: NEAR CAMERON MARSHALL COUNTY WEST VIRGINIA
     That he is pensioner under certificate No. 113453. That he has applied for pension under original No.---.
     That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
     That his post-office address is ROCK LICK, county of MARSHALL State of WEST VIRGINIA.

     I have the honor to acknowledge the receipt from your Office of application for Pension No. 158.896, and to return it herewith, with such information as is furnished by the files of this Office.
     It appears from the Rolls on file in this Office, that JESSE BANE was enrolled on the 14th day of AUGUST, 1862, at WHEELING, VA, in Co. "C", 12th Regiment of WEST VA Volunteers; to serve 3- years or during the war, and mustered into service as a PRIVATE on the 23rd day of AUGUST 1862, at WHEELING VA, in Co "C", 12th Regiment of WEST VA Volunteers, to serve 3- years or during the war. On the Muster out Roll of Co. "C" of that Regiment dated June 16" 1865, he is reported ABSENT WOUNDED IN ACTION AT FORT GREGG, VA, APRIL 2" 1865. In hospital at Fortress Monroe, Va.
DETACHMENT MUSTER OUT ROLL DATED JUNE 17"1865, REPORTS HIM MUSTERED OUT ON THAT DATE, AT FORT MONROE, VA.

On this the 10th day of May 1913 JESSE BANE A Pensioner under Pension certificate no 113453 Company C 12th Regt West virginia Vol Inft, Presented to me an old Family Bible, in which there are quite a number on dates of different occurances recorded, from the appearance of the writing therein it appears to me that it has been recorded there a long time and i fully believe that the book is the original family record of the father of said Soldier Jesse Bane, and that said record shows the following, JESSE BANE their son born december 5th 1842, this record bears the date 1830, and fairly well preserved, there are no errasures where the above date of birth is given, and i fully believe that the writing there in was recorded at or near the time of the occurance as stated,I have no interest in the claim,
