Declaration For Pension
Act of May 1, 1920.

State of WEST VA, County of WOOD, ss:
     On this 25th day of MARCH, A.D. one thousand eight hundred and ninety one, personally appeared before me, O. P. DUNHAM, Notary Public within and for the county and State aforesaid, JAS. PADGETT, aged 61 years, a resident of VALLEY MILLS, county of WOOD, State of WEST VA; who, being duly sworn according to law, declares that he is the identical JAMES PADGETT, who entered service during the War of the Rebellion under the name of JAMES PADGETT on or about the 22 day of August, 1862, as private in company D of the 12th regiment of W.Va. commanded by D. W. Blayney, and was honorably discharged at Richmond, on or about the 16" day of June, 1865. That his personal description is as follows: Age 61 years; height, 5 feet 8 inches; complexion, fair; hair, dark; eyes, blue. That while a member of the organization aforesaid, in the service and in the line of his duty at or near Winchester, in the State of Virginia, on or about the 15th day of July, 1863, he contracted chronic diarrhea an resulting weak back and hips.
That he was treated in hospitals as follows: Sharpsburg MD 1863, Sandy Hook MD July 1864, Anapolis MD 1864, Little York Pa 1864, Harris Landing Virginia March 1865.
That he has not been employed in the military or naval service otherwise than as stated above.
That since leaving the service his occupation has been that of FARMER. That prior to his entry into the service above named he was a man of good, sound physical health, being when enrolled a FARMER.
That he is now WHOLLEY disabled from obtaining his subsistence by manuel labor by reason of his injuries above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States.
     He hereby appoints,with full power of substitution and revocation, George E. Lemon, of Washington, D.C., his true and lawful Attorney, to prosecute his claim. That he has never received but has applied for a pension #812584. That his post office address is Valley Mills County of Wood, State of W.Va.
