DECLARATION FOR PENSION
THE PENSION CERTIFICATE SHOULD NOT BE FORWARDED WITH THE APPLICATION

     On this 19” day of November, A.D. one thousand nine hundred and ten, personally appeared before me, a Notary Public within and for the county and State aforesaid, William R. Hanna, who being duly sworn according to law, declares that he 76 years of age, and a resident of -------- county of Ohio, State of West Virginia; and that he is the identical person who was ENROLLED at West Liberty, West Va under the name of William R Hanna, on the 14” day of August, 1862 as a Private, in Co. “D” 12th Regt, West Va. Infantry in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at Richmond, Va, on the 16th day of June, 1865.

     That he also served no other service.

     That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet 0ne inches; complexion, Dark; color of eyes, Gray; color of hair, Dark; that his occupation was Farmer; that he was born July 16th, 1835 at Ohio County, West Virginia.

     That his several places of residence since leaving the service has been as follows: Washington County, Pa, and Ohio County West Va

     That he is a pensioner. That he has heretofore applied for pension Pensioned by Certificate No 705988. Act of February 6th 1907.

     That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.

     That his post-office address is West Alexander, county of Washington State of Pennsylvania. R.F.D. No. 4

Hanna, the claimant, sign his name (or make his mark) to the foregoing declaration; that they have every reason to believe, from the appearance of the claimant and their acquaintance with him of 11 and 40 years, respectively that he is the identical person he represents himself to be, and that they have no interest in the prosecution of this claim.

“William. R. Hanna, born July 16th. 1835. Is correct, and was copied into my bible, by my Mother. And that there have been no erasures, or alterations since copied.

Who had filed a claim, for Pension, under act of February, 6, 1907, Alleging that he is 76 years of age, and has furnished me with a copy of the family record, of which the following is a verified copy.
“William. R. Hanna. Born. July.16th.1835…The bible from which this record was taken, was printed in Philadelphia, by William. W. Harding. In 1868..The record bears no mark of erasure, nor alterations. and I believe all the records, in said bible, were written by the same person who, the pensioner, declares was his Mother. And were copied from the original family bible..The facts of which are set forth in the above affidavit..

Hanna, who being duly sworn according to law, declares that he is 76 years of age, and a resident of the state of West Virginia county of Ohio, State of West Virginia; and that he is the identical person who was ENROLLED at West Liberty West Virginia under the name of William R. Hanna, on the 14 day of August, 1862, as a Private, in Company D. -12 Regiment of West Virginia Infantry in the Civil War, and was HONORABLY DISCHARGED at Richmond Va., on the 16 day of June, 1865

That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet 1 inches; complexion, Dark; color of eyes, Gray; color of hair, Dark; that his occupation was Farmer; that he was born July16, 1835

Hanna, who being duly sworn according to law, declares that he is 85 years of age, and a resident of West Alexander county of Washington, State of Pennsylvania; and that he is the identical person who was ENROLLED at West Liberty, West Virginia under the name of William R. Hanna, on the 14th day of August, 1862, as a Private, in Company D. -12 Regiment of West Virginia Infantry Volunteers , in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Richmond Va., on the 16th day of June, 1865

That his personal description at enlistment was as follows: Height, 6 feet 1 inches; complexion, Dark; color of eyes, Gray; color of hair, Dark; that his occupation was Farmer; that he was born July16, 1835, at Ohio County Virginia (now West Virginia)

That he requires the regular personal aid and attendance of another person on account of the following disabilities: Infirmities incidental to old age.

That since leaving the service he has resided at: west Alexander, Pa and vicinity and his occupation has been farming.

That he is a pensioner under certificate No. 705988

That he makes this declaration for purpose of being placed on the pension roll of the United States under provisions of the act of May 1, 1920.
