SIR:
It is alleged that John Stollar enlisted August 6th, 1861 and served as a private in Co. "B" 1 Reg't W Va Cav also as a (blank) in Co. (blank) Reg't (blank) Commanded by Capt Brown and was discharged as Wheeling W Va, July 8, 1865. It is also alleged that while on duty at Winchested Va on or about October 16th, 1864, he was disabled by fever and ague, disease of stomach and malarial poisoning, rupture and weakness in back, rheumatism and was treated in hospitals of which the names, locations, and dates of treatment are as follows: ___________

In case of the above named soldier the War Department is requested to furnish an official statement of the enrollment, discharge, and record of service so far as the same may be applicable to the foregoing allegation, together with full medical history. Please give the rank he held at the time he is claimed to have incurred the disability alleged, and if records show that he was not in line of duty during that period, let the fact be stated.

DECLARATION FOR PENSION ACT OF FEBRUARY 6, 1907
THE PENSION CERTIFICATE SHOULD NOT BE FORWARDED WITH THE APPLICATION

County of York ss:

On this 28th day of February, A.D. one thousand nine hundred and seven, personally appeared before me, a notary public within and for the county and State aforesaid, John Stollar who, being duly sworn according to law, declares that he is 66 years of age, and a resident of Lushton, county of York, State of Nebraska; and that he is the identical person who was ENROLLED at Wheeling, West Virginia under the name of John Stollar, on the 6th day of August, 1861, at a Private, in Company B, First Regiment of Virginia Mounted Volunteers in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at Wheeling, West Virginia, on the 12th day of February, 1864.
That he also served in Co. B First Regiment of West Veteran Mounted Volunteers and discharged on July 8-1865 at Wheeling West Virginia.
That he was not employed in the military or naval service of the United States otherwise than as stated above.
That his personal description at enlistment was as follows: Height, 5 feet 8 3/4 inches; complexion, fair; color of eyes, blue; color of hair, black; that his occupation was farmer; that he was born May 17, 1840, at East Findley Township, Washington County, Nebraska.
That his several places of residence since leaving the service have been as follows; East Findley Township Washington County, Penn. until 1867. In Moulton, Iowa until 1871, at Lushton Nebraska since 1871.
That he is a pensioner. That he has heretofore applied for pension Pension Certificate No. 560999.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.
That his post-office address is Lushton, county of York, State of Nebraska

On this 20th day of May, A.D. one thousand nine hundred and Twelve, personally appeared before me, a notary public within and for the county and State aforesaid, John Stollar who, being duly sworn according to law, declares that he is 72 years of age, and a resident of Lushton, county of York State of Nebraska; and that he is the identical person who was ENROLLED at Wheeling, West Virginia, under the name of John Stollar, on the 6th day of August, 1861, as a Private, in Company B, First Regiment of Virginia Mounted Volunteers in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at Wheeling, West Virginia, on the 12th day of February, 1864.
That he also served Reenlisted to become a Veteran- at Wheeling , West Virginia, on the 4th day of February 1864 as Sergeant of Company B, First Regiment of West Virginia, Vet Mounted Volunteer and discharged at Wheeling West Virginia on the 8th day of July 1865.
That he was not employed in the military or naval service of the United States otherwise than as stated above.

That he is a pensioner under certificate No. 560999. That he has applied for pension under original No. 560999.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.

Gentlemen:

John Stollar of 367 Redondo Avenue, Long Beach, California, who had a pension of $ 50.00 per month which I believe was IC No. 560999, died in this city on the 21st day of October, 1927.

Payments of $50.00 per month for this pension were made up to and including October 1, 1927. No payment has been made for the period between the 1st of October and the 21st of October, 1927, the date of his death.

The executrix of his estate, Mrs. Zelma F. West, has requested me to make application for the amount of the pension which had accrued to the date of death.

His estate is in process of administration in Los Angeles County, California, and his daughter, Mrs. Zelma F. West, was appointed the executrix thereof, on the 5th day of December, 1927.
