Hospital Statement

I hereby certify that I am claimant for Pension No. 728743. I was late a member of Company I. 1st Regiment W Va Cav. Volunteers, and the following is a full, true, and correct statement of all treatment received by me while in the service of the United States, to the best of my recollection:
I was treated for the mumps at General Hospital Cumberland, Md about Jan or Feb 1862. I was in the Hospital at York Pa. about Sept 1864 for fever and Ague and yellow jander. I was also treated for Rheumatism, Liver complaint and General disability by Dr. Gray of Wind Ridge Pa about the spring of 1865 or 66 after discharge. I was Doctored by Dr. Porter of Harveys Pa during 1865 or 1866 for Rheumatism Liver complaint and janders and general disabilities. Also by Dr. J. T. Imis of Wind Ridge Pa. now of Waynesburg Pa. for general disability during the years of 1875 to 1880. I was then Doctored by John M. Murry of Wind Ridge Pa for general disability from 1880 to 1889. And also by Dr. Patterson of Harveys Pa during summer 1889.
Given this Sixteenth day of December 1889, and I further state that my Post Office address is Harveys County of Greene State of Pennsylvania

For Officer's or Comrade's Testimony

I, Peter Fry, a resident of White Collage in the county of Greene, State of Penna, late private of Company I of the first Regiment W Va Cav on oath, depose and say, that I was well acquainted with Daniel W. Sollars late a private in company I of the first Regiment W. Va. Cav. of the war of 1861: that at the time of his enlistment, said claimant was a sound man and while in the military service of the United States in the line of his duty, and without fault or improper conduct of his own, on or about the Thirtieth day of January 1862, at New Creek in the state of W Va, Said applicant was sent to the hospital at Cumberland MD with the mumps at about Sept 1864 the Said claimant was sent to the hospital at York Penna with Rheumatism and ague.
I know these facts from being present with the Company and I have no interest whatever in the prosecution of this claim for pension

On 15 Sept 1894, Daniel W. Sollars, 50 years old and a resident of Harveys, Greene Co, PA, appeared before a Justice of the Peace in Greene Co., PA. He declared that he was the identical Daniel W. Sollars who was enrolled on the 18th of Nov 1861 in Co. I, 1st W. Va. Cav. in the War of the Rebellion and was honorably discharged at Camp Russell, W. Va on the 5th of Dec. 1864. He claimed that he was unable to earn a support by manual labor by reason of injury to left knee. His post office address was Harveys, Greene Co, PA.

Declaration For Pension
Pension certificate No. 908934

In his application for a pension, dated 21 May 1912, Daniel W. Sollars swore before a Justice of the Peace that he was 68 years of age and a resident of West Finley, Greene county, Pennsylvania and that he was the identical person who was enrolled at Wheeling, West Virginia under the name of D. W. Sollars on the 18 day of November 1861 as a Private in Co. I, First West Virginia Volunteer Cavalry and was honorably discharged at Russell, West Virginia the fifth day of December 1864. He stated that his height was 5' 11", complexion dark, eyes gray, hair color dark. He was born 10 January 1844 at Harveys, Greene Co., Pennsylvania. He made this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912. His post office address was West Finley, Washington Co., Pennsylvania.

Declaration For Widow's Pension
State of Pennsylvania, County of Greene

On this 17 day of March, 1921, personally appeared before me, a Justice of the Peace, within and for the County and State aforesaid, Mary H. Sollars, who, being duly sworn by me according to law, declares that she is 77 years of age and that she was born Aug 28, 1843.
That she is the widow of Daniel W. Sollars, who enlisted November 18, 1861 at Wheeling, West Virginia, under the name of Daniel W. Sollars as a Private, in Company I, West Virginia volunteer Cavalry and was honorably discharged December 5, 1864, having served ninety days or more, or was discharged for or died in service of a disability incurred in the service and in the line of duty, during the Civil War.
That she was married to said soldier December 10th, 1870, under the name of Mary H. Messinger, at Rymons Station Greene County, Penna. by Reverand Stillwagon; that she had not been previously married; that he had been previously married to Hannah A. Bogard in 1863, April 7th. Died April 21, 1870.
That said soldier died March 7, 1921, at West Finley, Penna. R.D. that she was not divorced from him; and that she has not remarried since his death.
The following are the ONLY children of the soldier under sixteen years of age, who are now living-NONE She has not heretofore applied for pension....the number of his pension certificate being 908-934.

DIRECTIONS:
If you go to Graysville, Pa and turn up Ackley Creek RD. and follow it to the county line there will be a road on the right that is called Enon Church RD. on your left will be a large white house well kept up. The cemetery is on the right where Enon Church Rd and Ackley Creek Rd meet. It can not be seen from the road. The best way to access it is to turn up Enon Church Rd and you will see a small cottage on the right, to the right of the cottage is a path going up the hill and it is up on top of the hill.

1. LEVI1 SOLLARS was born April 17, 1810, and died April 25, 1892 in Sollars cemetery, Greene Co., Pa.. He married ELIZABETH BURNS, daughter of JAMES BURNS and ELIZABETH JOHNSON. She was born July 28, 1825, and died August 25, 1889.
