DECLARATION FOR THE INCREASE OF AN INVALID PENSION

State of West Virginia

On this 6 day of March A.D., one thousand eight hundred and seventy-eight, personally appeared before me Clerk of Circuit Court Ohio Co. W.Va. the same, being a Court of record within and for the County and State aforesaid, William A. McCoy aged 38 years, a resident of Wheeling County of Ohio, State of West Virginia, who, being duly sworn according to law, declares that he is a pensioner of the United States, duly enrolled at the Knoxville, Tenn., Pension Agency at the rate of Seven 50/100 dollars per month, by reason of disability incurred in the Military service of the United States while (1) Captain of Co. C 1st Regt. West Va. Cavalry Vols. that his present physical condition is such that he believes himself to receive an increased pension; and that he herewith returns his present pension certificate.
He further declares that he is disabled in the following manner, to wit: (2) Gunshot wound for the left breast which has become more aggravated and further increase his disability.

That he appoints GEO. BANCROFT & CO., of Washington, D.C., his true and lawful attorneys to prosecute his claim; that his residence is No. 87, in Maryland Street, of Wheeling, County of Ohio and State of West Va.; and his post office address is same. (Attest)-- Two witnesses who can write R. G. Ban

On this 10th day of August, A.D. one thousand eight hundred and eighty-nine personally appeared before me, a Clerk of Circuit Court within and for the county and State aforesaid, William A. McCoy, aged 49 years, a resident of the town of Triadelphia, county of Ohio, state of West Virginia, who, being duly sworn according to law, declares that he is a pensioner of the United States, enrolled at the Washington, D.C. Pension Agency at the rate of $20.00 dollars per month, by reason of disability from Gunshot wound of left breast and resulting disease of lungs incurred in the Military service of the United States while Captain of Co. "C". 1st Regt. West Virginia Volunteer Cavalry.
That he believes himself to be entitled to an increase and re-rating of pension on account of having originally been rated too low. Increase of disability for which already pensioned. Resulting rheumatism in left arm, shoulder and side. And partial paralysis of left arm totally incapacitating him for any kind of manual labor, that he appoints_______(blank), of _______(blank) county of ______(blank), state of ________(blank), his true and lawful attorney, to prosecute his claim. OH THIS 6" day of June, A.D. 1901, personally appeared before me a Notary Public, in and for the aforesaid County, duly authorized to administer oaths J. C. McCurdy aged 53 years, a resident of Elm Grove, in the County of Ohio, and State of West Va. whose post-office address is Elm Grove, West Va. well known to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows:
" I have known and have been personally acquainted with the above claimant, Sevena R. McCoy, for the past 30 years. She has not remarried since the soldiers death. She owns her own home and lives in it which is assessed at $1000.00, on which there is a deed of trust for $650.00, her personal property was so insignificant that the assessor refused to assess her with any, partly for that reason and partly because she was a soldier's widow. She owns no other property, real, personal or mixed, nor has she had any interest in any form which she derived any income since March 23, 1901. Her only means of support is by her daily labor, and she has had no income from any source outside of her daily labor since March 23, 1901. This statement is made from personal knowledge and further declares that he has no interest in said case and is not concerned in its prosecution.

Hutchison aged 58 years, a resident of Woodsdale, in the County of Ohio, and State of West Va. whose Post-office address is Woodsville, Ohio Co., West Va. well known to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows: "I have known and have been personally acquainted with the above claimant, Sevena R. McCoy, for the past 40 years. She has not remarried since the claims death. She owns her own home and lives in it which is assessed at $1000.00, or which there is a deed of trust for $650.00. West Va. well known to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows: " In reply to letter dated April 20, 1901. Say: I own my little home situated in Woodsdale, Ohio County, West Va., assessed at $1000.00, my personal property was so small that the assessor did not see fit to assess me for any amount, I own no other property, real, personal, or mixed, including bonds, stocks and investments, not have I had any interest in any from which I derive any income. I have had no income form any source outside of my daily labor since March 23, 1901. There is no one legally bound to provide for my support.

On this 28th day of June, A.D. one thousand eight hundred and eighty-four personally appeared before me, a Clerk of the Circuit Court within and for the county and State aforesaid, Wm. A. McCoy, aged 44 years, a resident of the town of Tridelphia, county of Ohio State of West Virginia, who, being duly sworn according to law, declares that he is a pensioner of the United States, enrolled at the Knoxville, Tenn. Pension Agency at the rate of 13.00 dollars per month, by reason of disability from gunshot wound in left breast incurred in the Military service of the United States while Captain of Company "C", 1st Regiment, West Virginia Vol. Cavalry. That he believes himself to be entitled to an increase of pension on account of increase of the disability for which he is now pensioned. Hemorrhage of the lungs more frequent, partial paralysis of left arm all the time and part of the time, total. Constant pain in left shoulder and breast at times very severe.

PHYSICIAN'S AFFIDAVIT
State of West Virginia County of Ohio, ss:

In the Pension Claim No. 715.461 of Sevena R. McCoy; widow of William A. McCoy, late a Capt. Co. "C" 1" Regt. West Va. Cavalry.

Personally came before me, a Notary Public, in and for the aforesaid County and State J. A. Cox whose Post Office address is Beabody Building, Wheeling, West Va. well known to me to be reputable and entitled to credit, and who being duly sworn, declares in relation to the aforesaid case as follows:
That he is a practicing physician, and has been acquainted with the above-named soldier for about 30 years, and that said Wm. A. McCoy came under his professional care Aug. 23, 1899, and continued under his care until his death March 4, 1900. Affiant declares that upon examination he found his patient, said Wm. A. McCoy, slowly recovering from an abscess of the left lung caused as he believes by a gunshot wound received in late Rebellion.
As a result of this wound, affiant states that Capt. McCoy has from time to time suffered from (illegible) hemorrhages of the lungs. Affiant states that a slow fatty degeneration of heart was setting in attended with swelling of limbs and body. Affiant declares that while said Wm.A McCoy died from a complication of disease, he believes firmly that they were super-induced by the wound, and that had it not been for the wound, the Captain would today be in the enjoyment of health and strength.


State of West Virginia, County of Ohio:

I, Henry Capehart, hereby solemnly swear and make oath that I reside in Bridgeport, Belmont County, State of Ohio, that my post office address is Bridgeport, Belmont County, State of Ohio, that I have no interest whatever, direct or indirect, in the application No. 163,201 for an invalid pension from the United States of America of William A. McCoy, late Captain of Co. "C", 1st West Va. Cavalry, nor in the prosecution thereof; that I have personal knowledge as hereinafter stated that William A. McCoy while in the line of his duty as soldier on the 3rd of May, A.D. 1863 as Captain as aforesaid in battle at Warrenton Junction, Virginia, was struck by a musket ball in the left breast just above the heart, the ball passing through the body and being taken out of the back. I was at said date Surgeon of the said regiment and was on duty at Gen. Stahl's headquarters, either as Medical Director or Surgeon-in-Chief of Division, which not certain now, at Fairfax Court House, Virginia. The wounded were brought there directly from the battlefield the first train and gave said McCoy immediate attention. There were some 27 rebel wounded and prisoners brought down with McCoy from Moseby's Command. Surgeon Woods of the 5th N. Y. Cavalry who attended McCoy on the field is I suppose, dead as he was very delicate health the last I saw of him. His whereabouts if alive are to me unknown.

A. McCoy late Capt. Co "C" 1st West Va. Cavalry for a invalid pension from the United States nor in the prosecution thereof; that the alteration at "The" was made, and the (illegible) at "the first" was made, and "train" was interlined, and "My post office address is Bridgeport, Belmont Co. Ohio" was added, all before signing, swearing or execution.

DECLARATION FOR ORIGINAL PENSION OF AN INVALID

State of West Virginia; County of Ohio: ss:

On this Eleventh day of February A.D. one thousand, eight hundred, and seventy-one, personally appeared before me, Samuel B. McColloch, a duly authorized officer (Clerk) of a court of record, in and for the county and state aforesaid William A. McCoy, who being duly sworn according to law declares, that he is the identical William A. McCoy in the military service of the United States at Washington, Washington County, Pennsylvania on the 24th day of April in the year 1861, as a musician in Company C, 12th Regiment of Pennsylvania Infantry Volunteers, three months men, in the war of 1861, and was honorably discharged therefrom on the 4th day of August in the year 1861; that he again enlisted in the military service of the United States under the same name at Washington, Washington County, Pennsylvania on the 6th day of August, 1861 as a First Sergeant in Co, B, 1st Regiment Virginia, (afterwards West Virginia), Cavalry Volunteers in the war of 1861 and was honorably discharged therefrom to date June 29th 1862 to accept promotion to the rank and office of 1st Lieutenant of Co. C., of the same regiment and was mustarded in as such on or about the 30th day of June, 1862, under the same name, in the war of 1861, and served as such till discharged honorably as such October 13th 1862 to accept promotion to the office.

It is hereby certified That William A. McCoy formerly a Captain of "C" Company, in the 1st Regiment of West Va. Cavalry, in the war of the Rebellion who is now paid at the Wheeling W.Va. Agency at the rate of 15 dollars per month, on account, as he states, of Gunshot wound of left breast and it's results while in the line of duty in the military service of the United States, on or about the third day of May, 1863, at a place called Warrenton Junction, in the State of Virginia is still suffering in consequence of said wound.
The disability originates entirely from the injury or disease on account of which he was originally pensioned, as follows: Height, 6 ft. 1 3/4 in. Weight, 220 lbs; complexion, dark, age 36; respiration, 28; pulse 72
Ball entered left breast two inches behind nipple and emerged at infusion portion of left scapula with loss of it's substance. Circulation healthy. Dullness over (illegible) of wound brachial respiration; Applicant alleges that his sufferings have gradually increased since the fall of 1875 at which time he had typhoid fever. Since which date he has been entirely disabled from engaging in active employment. Exercise producing (illegible) and constant exhaustion. (illegible) has occurred none frequently "three within a month". He is (illegible) with many cough. Suffers pain in the left shoulder irritated before (illegible) of blood.

We find his disability above, to be equal to, and entitling him to: Three eighths 750 which we recommend.
