NOTE-- This can be executed before any officer authorized to administer oaths for general purposes. If such officer uses a seal, certificate of Clerk of Court is not necessary. If no seal is used, then such certificate must be attached.
State of Penna, County of Greene, SS:
On this 19th day of July, A.D. one thousand eight hundred and ninety, personally appeared before me, a Justice of the Peace within and for the County and State aforesaid, Wind Ridge Pa. James Ackley aged 50 years, a resident of the Township of Richhill, County of Greene, State of Penna, who being duly sworn according to law, declares that he is the identical James Ackley who was enrolled on the 18th day of August, 1862; in Private Company K 16th Regiment Pa. Cavalry in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARGED at Lynchburg Va. on the 17th day of June, 1865. That he is unable to earn a support by reason of Diabeties, heart disease, a loss of left eye. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. That he has not applied for pension under application NO. ____. That he is no pensioner under certificate NO. ____
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. He hereby appoints T.C. Pipes of Camerron, W.Va. his true and lawful attorney to prosecute his claim. That his Postoffice Address is Greene County Pa.

On this 11th day of March, A.D. one thousand nine hundred and ten, personally appeared before me, a NOTARY PUBLIC within and for the county and state aforesaid, James Ackley, who being duly sworn according to law, declares that he is _____ years of age, and a resident of West Finley county of Washington, State of Pennsylvania and that he is the identical person who was enrolled at West Finley Washington Co. Pa. under the name of James Ackley, on the 18th day of Aug, 18__ as a corp'l in K 16th Pa Cav in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Lynchburg, Va, on the 17th day of June, 1865. That he also served ______
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height Five feet 7 inches: complexion, sandy; color of eyes, gray; color of hair, sandy; that his occupation was farmer; that he was born March 10, 1840 at Greene County Pa.
That his several places of residence since leaving the service have been as follows: Richhill Township Greene Co. Pa. and West Finley Township Washington Co. Pa.

I Annie Ackley do solemely swear that I am the widow of James Ackley Dec'd whom died Feb. 12, 1914 at his home in WestFinley, Pa., was married to James Ackley Oct. 5, 1870 at Claysville Pa. by Rev. Alexander McCarroll and never received any marriage certificate. The only witness to marriage was Rev. Alexander McCarroll and Henry Ackley whom are both dead.

DECLARATION FOR WIDOW'S PENSION

On this 20th day of Feby, A.D. one thousand nine hundred and Fourteen personally appeared before me, a NOTARY PUBLIC, within and for the county and State aforesaid, Annie Ackley, aged 69 years a resident of West Finley, county of Washington, State of Pennsylvania, who, being duly sworn according to law, makes the following declaration in order to obtain pension under the provisions of the ACT OF CONGRESS APPROVED APRIL 19, 1908.
That she is the widow of James Ackley, who was enrolled under the name of James Ackley, at ____ on the 18th day of Aug, 1862 as a corp'l in Co. K. 16th . Pa. Cavalry, and honorably discharged 17th day of June, 1865, having served ninety days or more during the late civil war.
That he also served ______
That she was married under the name of Annie Potter to said soldier at Claysville Pa., on the 5th day of October, 1870, by Alexander McCarroll (Minister) that there was no legal barrier to the marriage; that she had not been previously married; that the soldier had not been previously married,_______ and that neither she nor said soldier married otherwise than as stated above.
That the said soldier died Feb. 12, 1914, at West Finley Pa.,; that she was not divorced from him, and that she has not remarried since his death. That the said soldier left the following-named children who are now living and under sixteen years of age, to wit:
