Declaration for Invalid Pension

State of Pennsylvania, County of Greene, ss:
     On this 24 day of July one thousand eight hundred and ninety personally appeared before me, a Justice of the Peace within and for the County and State aforesaid, John S. Ackley aged 60 years, a resident of the township of Rich Hill County of Greene, state of Pennsylvania, who, being duly sworn according to law, declares that he is the identical John S. Ackley who was enrolled on the 8th day of September, 1862, in Co. C 18th Pa. Cavl. Vols. ninety days, and was HONORABLY DISCHARD at Cumberland, Md., on the tenth day of July, 1865. That he is almost totally unable to earn a support by reason of weak back, diabetes, liver trouble, scurvey, kidney trouble. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. That he has never applied for pension under application No.--------. That he is not a pensioner ........... That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. He hereby appoints J. W. Mathews of Cameron, Marshall Co., W. Va. his true and lawful attorney to prosecute his claim, and he directs that the sum of ten dollars be paid him for his services.

That his post office address is Video county of Greene state of Pennsylvania.

Declaration for Pension

State of Pennsylvania
County of Washington
     On this 28th day of February, one thousand nine hundred and seven personally appeared before me, a Justice of the Peace within and for the county and State aforesaid, John S. Ackley, who, being duly sworn according to law declares that he is 79 years of age, and a resident of Richhill Township county of Greene State of Pennsylvania; and that he is the identical person who was enrolled at Pittsburgh under the name of John S. Ackley, on the 8th day of Septemeber 1862, as a C. M. Sergeant in Captain Frederick Zarrachers Company (C) 18 Regiment of Pennsylvania Cavalry Volunteers in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at Cumberland, Maryland on the 10th day of July, 1865.

That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet 9 inches; complexion, light; color of eyes, brown; color of hair, brown that his occupation was farmer; that he was born January 10th, 1828, at Greene County.

     That his several places of residence since leaving the service have been as follows: only in Richhill Township, Greene County, Pa.

     That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.

     That his post-office address is West Finley R. D. 2, county of Washington, State of Pennsylvania.

Ackley, residing in Greene County, persons of whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say that they were present and saw John S. Ackley, the claimant, sign his name to the foregoing declaration; that they have every reason to believe, from the appearance of the claimant and their aquaintance with him of 25 years and 25 years, respectively, that he is the identical person he represents himself to be, and that they have no interest in the prosecution of this claim.

Supplemental Declaration for Invalid Pension

State of Pennsylvania, County of Greene, ss:
     On this 26 day of June one thousand eight hundred and ninety seven personally appeared before me, a Justice of the Peace within and for the County and State aforesaid, John S. Ackley aged 69 years, a resident of the P. O. of Video County of Greene, State of Pa., who, being duly sworn according to law, declares that he is the identical John S. Ackley who was enrolled on the --- day of Nov., 1862, in Co. "C" 18 Reg. Pa. Vol. Cavy. as Comy Sergt. in the service of the United States in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARED at Cumberland, Md., on the 10 day of July 1865. That he was disabled for earning a support by manual labor in a pensionable degree on June 27, 1890 & on the date of filing his original declaration, by reason of the following disabilities: piles & loss of teeth from scurvy. That he is also disabled for earning a support by general debility caused from scurvey as well as from age of general....

That said disabilities are not due to vicious habits, and are to the best of his knowledge and belief of a permanent character, and that he is now wholly disabled for earning a support by manual labor in consequence of same. That he has applied for pension under application No...... That he is a pensioner under Certificate No. 775,084. That he has not been employed in the military or naval service otherwise than as stated above. That he makes this declaration for the purpose of his claim and being placed on the pension roll of the United States, under the provisions of the Act of June 27, 1890. He hereby appoints Robt. A. Sayers of Waynesburg, Greene Co., Pa. his true and lawful Attorney, to prosecute his claim, and he directs that the sum of Ten Dollars be paid to said Attorney.
