DECLARATION FOR PENSION

THE PENSION CERTIFICATE SHOULD NOT BE FORWARDED WITH THE APPLICATION

State of West Va, County of Marshall,ss:
On this 5 day of November, A.D. 1914 personally appeared before me, a Notary Public within and for the said county and state aforesaid, Edward GRANDON, who being duly sworn according to law, declares that he is now 66 years of age, and a resident of Moundsville, county of Marshall, State of West Va; and that he is the identical person who was ENROLLED at Wheeling West Va, under the name of Edward GRANDON, on the 24 day of February, 1864, as a Private in Company H First West Va Infantry in the service of the United States, in the Civil was, and was HONORABLY DISCHARGED at Clarksville West Virginia (Note: underneath this is written with Company), on the 16 day of July, 1865, That he also served then consolidated with Company F 2nd regiment West Va veterans to which transferred from Co H West Va Volunteers this is taken From certificate of loss of original discharge was given at War dept Washington D C. the 22 day of july 1890

That he was not employed in the military or naval service of the united states otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet, 5 1/2 inches; complexion, Fair; color of eyes, Brown; color of hair, Brown; that his occupation was Farmer; that he was born March 4th, 1848, at in Greene Co Pennsylvania on a farm. (If it is desired to claim under the disability clause, the nature of the disability and the date, place, and cicumstance under which it was incurred in line of duty should be alleged in this space. A disability which did not originate in the service and in line of duty can not be considered.)
NOTE: Those lines are blank.

That his several places of residence since leaving the service have been as follows: in Washington Co Pa and in Moundsville West Va have lived here for the last three years ( State date of each change, as nearly as possible.)

That he is a pensioner under certificate No. 602.1(or a c)37 That he has not applied for increase of pension under original No. 602c37

That he makes this declaration for the purpose of being placed on the pension roll of the United states under the provisions of the act of May 11, 1912. That his post-office address is Moundsville, county of Marshall State of West Va

NOTE: I also have the Widow's Pension paper and some others from this file. He was married to Levina MILLER, never remarried, never divorced. They were married on July 12, 1866. He died April 7, 1918.
